Pre-Appointment Checklist
Please complete these steps 2 business days before your appointment:
- Forms: Complete all assigned questionnaires in the "Questionnaires" tab.
- Records: Upload an updated Medication & Supplement list.
- Billing: Ensure your Credit Card on file is current.
Cancellation Policy
- 72-Hour Notice: To avoid a fee equal to 100% of the visit cost, please cancel or reschedule at least 3 business days in advance. (e.g., a Monday 9 AM appointment must be cancelled by Tuesday 9 AM the week prior).
Portal Messaging Guidelines
Use for brief, non-urgent questions. Please allow 5 business days for a reply.
- Refills: Request via the portal at least 7 days before running out.
- Clarifications: Questions regarding your last visit (within 7 days of the appt).
- Note: New symptoms or complex changes require a follow-up appointment. We do not provide clinical consultations or personalized medical advice via the portal.

To ensure you get the most out of your time, we recommend taking your own notes. A formal visit summary may not be available immediately following the consultation.
